1. Voluntary Participation & Wellness Confirmation: As the parent or legal guardian of the minor named above, I give permission for my child to participate in in-person (San Diego partner studios) or live online (Zoom) yoga, movement, and mindfulness classes with Growing with Yoga.

2. Safe Space & Medical Release: I understand that yoga involves physical movement and stretching. I confirm my child is healthy to participate, and I assume ordinary risks associated with recreational movement activities, releasing Growing with Yoga and its instructors/partner venues from liability to the fullest extent permitted by California law.

3. Emergency Care: In the event of an in-person medical emergency where I cannot be reached immediately, I authorize instructors to secure necessary first-aid or medical care.
